The Fresno State Sports Medicine program is committed to providing exceptional services to the student-athlete by utilizing a highly qualified, diverse and motivated staff whose foundation is built on the utmost ethical standards. Through communication, creativity and innovation, the athletic training staff, team physicians and allied specialists will collaborate in a team approach in order to meet the needs of the student-athlete. The certified athletic training staff will be aggressive in pursuing its own continuing education so as to remain on the cutting edge of the sports medicine field, allowing the department to provide the best available health care to its student-athletes.

Our student-centered approach to healthcare utilizes a multi-disciplinary team of providers to meet the needs of our student-athletes. This multi-disciplinary team includes Team Physicians, Athletic Trainers, Sport Dieticians, Behavioral Health Specialists, Chiropractors, Massage Therapists, Orthotists, Sport Psychologists and Physical Therapists. These disciplines assist us in helping our student-athletes perform to their highest ability in all aspects of life. This includes, but is not limited to:
- Pre-participation physical exams
- Movement assessment and corrective exercise screenings
- Injury evaluation, management, and rehabilitation
- Identification and referral to appropriate specialty areas
- Provide emergency care for injuries
- Injury prevention programs
- Identification of any predispositions to injury
- Design and implementation of Emergency Action Plans
- Safe return to practice and competition
- Recognition and referral for nutritional concerns
- Collaborating to ensure both practice and conditioning activities are completed in a safe environment
- Any other matter involving the health or safety of the student-athlete
What to expect on your physical date

The purpose of this page is to help both current and prospective student-athletes meet the medical eligibility requirements set forth by the NCAA and Fresno State Athletics. Your physical examination will be conducted in the Meyers Family Sports Medicine Center in the Student-Athlete Village off Bulldog Lane. The date and time will be given to you by your sport coach and athletic trainer. If you do not have the appropriate paperwork completed upon arrival, you will be asked to leave and come back at another date.

Prior to your physical date, you are required to complete the following:

- Open email from your athletic trainer and follow the link and steps to create your student portal account on healtheathlete
- If you have not received the healtheathlete portal link via email, please contact your teams athletic trainer immediately and we will resend the invitation
- Complete all forms and waivers as described in the instructions
- Please fill out your Pre-Participation Medical History Questionnaire accurately and honestly. To do this, you may need your parent or guardian’s help with dates and information, but it is important that you complete the form yourself so that we have accurate information pertaining to specific physical symptoms you may have experienced while participating in sports.
- It is important to be aware of the NCAA Medical Exception Policy for Banned Drugs. It is important to note that most ADHD medications as well as some hormone therapy medications fall under this rule and require specific documentation to continue taking as a student-athlete. It is your responsibility to bring this documentation with you.
- Upload a photo of the front and back of your insurance card to your healtheathlete portal account. If you need assistance, please follow the healtheathlete instruction sheet on proper uploading.
- Make a copy of your sickle-cell test results to bring with you on the day of your physical
- If you have never been tested, you can go to a local clinic and request a test—it is an NCAA requirement for clearance.
- If you are unable to complete one prior to your physical date, you can have the test completed here as part of your physical—just note that waiting for the results can delay your clearance.
- If under the age of 18, bring a signed minor consent form from your legal guardian
- Form will be available on your healtheathlete student portal account
- When completing the rest of your paperwork, please be sure to have your legal guardian sign in all the appropriate locations. These are highlighted in all forms when required.
The following is an example schedule of the day of your physical, which is split into two sessions.

- Check-in and paperwork review
- Height and weight
- Blood pressure and pulse
- Vision Assessment
- Electrocardiogram
- Lab work
- Nutrition Assessment
- Behavioral Health Screening
- Isokinetic strength testing (if necessary)
- Concussion Baseline Screening
Approximately 5:30pm-7:00pm
- General Medical Exam with Team Physician
- Orthopedic Exam with Team Physician
- Check-out and final paperwork review
